Attorneys-at-Law requesting an official name change on record must submit the following documents to the Registrar of the Supreme Court for verification and approval.

Required Documents

1.   Formal Request Letter
3. Supporting Documents Based on the Type of Name Change:
  • Submit the amended original Birth Certificate with the change recorded under Column 13.
    • Attach an official English translation certified by a sworn translator.
  • Submit the original Marriage Certificate.
  • Attach an official English translation certified by a sworn translator.
4. Copy of Certificate of Enrolment as an Attorney-at-Law

Submission Instructions:

  • All documents must be submitted physically to the Supreme Court Registry or scanned and emailed to: 📧 scurgentmatters@sltnet.lk
  • Please use the email subject line: Name Change Request – [Your Enrolment Number]

Important Notes:

  • Incomplete submissions or lack of supporting documents may cause processing delays.
  • Name change updates will be officially recorded upon verification and approval by the Supreme Court
